Basic Care Plan Companion
Sitting Services:
$30/HOUR
- Assist with daily activities and personal care: This includes helping with bathing, dressing, grooming, and other personal hygiene tasks.
- Remind and assist with medication schedules: Ensuring medications are taken on time and as prescribed.
- Prepare nutritious meals: Focusing on the dietary needs and preferences of the client.
- Perform light housekeeping duties: Such as cleaning, laundry, and maintaining a safe living environment.
- Accompany to appointments and run errands: Providing transportation and assistance with external activities.
- Assist with mobility and prevent accidents: Including transferring, positioning, and fall prevention strategies.
- Engage in recreational activities: Activities tailored to the client’s interests to keep them mentally and physically stimulated.
- Monitor health and safety, report any concerns: Keeping a vigilant eye on the client’s well-being and communicating with healthcare providers as necessary.
- Document and report daily activities: Keeping accurate records of care provided and the client’s condition.
- Communicate and coordinate with relevant parties: Ensuring a collaborative approach to care by staying in touch with family members and healthcare professionals.
- Offer emotional support and comfort: Being a compassionate companion to clients.
Level 1: Basic Assistance
- Services:
Incorporates general services with a focus on light housekeeping, meal preparation, companionship, medication reminders, and assistance with walking. This level is designed for individuals who are mostly independent but require some support with daily tasks.
Advanced Care Plan

All Levels Include Basic Care Plan Companion Sitting Services:
Essential companion sitting services are provided at every level of care, focusing on comfort, social interaction, and basic needs. This foundational support includes engaging in conversation, recreational activities, and companionship to enhance life quality.
Level 2: Enhanced Personal Care – $32/hour
Services Description: Extends foundational services to offer comprehensive personal care assistance for individuals requiring additional support with activities of daily living (ADLs) such as bathing, dressing, and grooming. Includes escorting clients to appointments and engaging in tailored companionship activities, suitable for those who need more assistance yet retain some independence.
Level 3: Advanced Assistance – $35/hour
Services Description: Adds to Level 2 by providing further support for clients with greater mobility needs due to pronounced physical limitations or health conditions. This level focuses on enhanced health monitoring, meticulous medication administration(administered by RN), and assistance with therapeutic exercises, targeting clients who require thorough care for their safety and well-being.
Level 4: Comprehensive Care for Bedbound Clients – $38/hour
Services Description: Offers exhaustive care for individuals who are bedbound, incorporating all services from previous levels with specialized attention. Focuses on advanced ADL assistance, rigorous health monitoring, strict medication management(administered by RN), and constant companionship, designed for clients who need continuous, round-the-clock care due to severe health challenges.
Skilled Nursing
STARTING AT
$45/HOUR

Vital signs monitoring
Medication administration
Pricing: $30 – $38 per hour
Services include light housekeeping, meal preparation, companionship, and medication reminders. Skilled nursing is not included at this level.
Level 2: Intermediate Care
Pricing: $45 – $50 per hour
Services expand to include Level 1 services plus assistance with personal care. Basic health monitoring by a licensed nurse is included, focusing on non-complex medication management and other basic nursing services.
Level 3: Advanced Care
Pricing: $51 – $60 per hour
At this level, services cover more comprehensive personal care and the addition of skilled nursing services, including the $45 medication management, wound care, and chronic illness management. This level is designed for clients requiring more frequent and complex nursing interventions.
Level 4: Comprehensive Care
Pricing: $61 – $75+ per hour
This level offers full personal care services and extensive skilled nursing care. It’s tailored for clients with significant health issues requiring intensive, skilled care. Services include managing complex medical conditions, coordinating care with healthcare professionals, providing patient and family education on health condition management, and providing detailed monitoring.
Post Surgery Care
